Carroll Lutheran Village in Westminster, MD is seeking a dedicated Full-Time RN Care Coordinator to join our mission-driven team. The role involves coordinating care for residents, educating families, and linking residents with community resources in a not-for-profit setting.

Key duties include developing care plans, following up after hospital stays, and serving as a clinical resource for Residential Living. RN license in Maryland and CPR certification are required; BSN is preferred.

Qualifications

• RN license in Maryland (active)
• BSN degree preferred
• Minimum 2 years of RN experience in acute care, long-term care, or community settings; LTC or assisted living preferred
• Healthcare Provider CPR certification required

Responsibilities

• Develop and coordinate the community's Care Coordination program.
• Identify and engage high-risk residents; serve as primary clinical resource for Residential Living.
• Create individualized care plans with physicians and healthcare team.
• Conduct post-hospital and ER follow-ups; review discharge instructions and arrange appointments.
• Provide basic clinical services: BP checks, dressing changes, glucose monitoring.
• Act as liaison between residents and healthcare providers including specialists.
• Guide residents and families through healthcare system; connect with community resources.
• Advocate for access to appropriate care and services.
• Respond to emergency alerts and follow up with unwell residents.
• Perform comprehensive nursing assessments and support disease self-management.
• Participate in interdisciplinary care planning and goal setting.
• Document all resident interactions and outcomes.
• Coordinate with hospital navigation and community health teams.
• May serve as Employee Wellness Nurse; coordinate pre-employment physicals and safety processes.
• Participate in continuing education and serve on committees.
