Job summary

Montana Department of Corrections seeks an RN Care Coordinator at Montana State Prison in Deer Lodge, MT. The role supports medical management for over 1,600 inmates in a 24-hour infirmary, including assessments, triage, medications, and chronic care management.

Responsibilities include coordinating transport, liaising with providers and telehealth, conducting chart reviews, educating patients, and ensuring timely follow-ups and discharge planning.

• Work with infirmary nurses on pre-op and post-op admissions.
• Conduct chart reviews for referrals and locate ICD-10 codes as needed.
• Gather off-site reports and monitor internal clinic follow-ups.
• Assist clinic staff as needed.
• Serve as point of contact for on-site VA appointment scheduling.

As a Registered Nurse at Montana State Prison, you will play a vital role in the medical management of over 1,600 inmates within a 24‑hour infirmary unit. This position supports comprehensive nursing functions, including assessments, triage, sick call, medication administration, mental health support, urgent and emergent response, chronic care management, infection control, quality improvement, and discharge planning.

QUALIFICATIONS

The knowledge, skills and abilities for this position are typically acquired through a combination of education and experience equivalent to:

Licensed to practice as an RN in the State of Montana is required.

Alternative combinations of education and experience will be considered on a case‑by‑case basis.
