Mayo Clinic in Minnesota is seeking an experienced Registered Nurse (RN) to support patients with chronic conditions transitioning from hospital to primary care. You will conduct post-hospital follow-up calls and in-home visits, coordinating care plans with primary providers to prevent readmissions.

This role emphasizes patient education, care coordination, and education on medications and treatment plans, with flexibility to adapt to varying schedules and locations within Mayo Clinic sites.

Qualifications

• Graduate of a baccalaureate nursing program, as recognized by ACEN/CCNE/NLN CNEA.
• If graduated from a nursing program not ACEN/CCNE/NLN CNEA accredited, at least one year RN experience or one year LPN experience at Mayo Clinic is required.
• Within five years from RN start date, evidence of baccalaureate degree completion may be required for some roles.
• Current RN license by state requirements; AZ/FL requires BLS; Midwest requires BLS for Health Care Providers.
• Additional state licensure or specialty certification/training may be required by the work area.

Responsibilities

• Post-hospital follow-up calls for patients at risk of readmission.
• In-home visits for adults receiving coordinated medical care after discharge.
• Partner with primary care providers to develop, implement, and monitor care plans.
• Manage a defined patient panel with ongoing follow-up and care coordination.
• Educate and support patients about chronic conditions, treatments, and medications.
