Job summary

Mayo Clinic is seeking an experienced Registered Nurse (RN) in Minnesota to join a specialized primary care team focused on patients with chronic conditions transitioning from hospital to home. You will perform post-hospital follow-up, in-home visits, and collaborative care planning to reduce readmissions and improve outcomes.

The role requires a BSN, valid RN license, and strong communication and leadership skills.

Qualifications

• RN with BSN from an accredited program.
• Experience caring for patients with chronic conditions.
• Excellent communication and teamwork.
• Current RN license in applicable state; Basic Life Support (BLS) competency.

Responsibilities

• Post-hospital follow-up calls for patients at risk of readmission.
• Home visits for adult medical care coordination after discharge.
• Collaborate with primary care providers to implement care plans.
• Manage a defined patient panel with ongoing follow-up and coordination.
• Educate and support patients on chronic conditions and medications.
