Mayo Clinic is seeking an experienced Registered Nurse (RN) to join a Primary Care team focused on patients with chronic conditions transitioning from hospital to primary care. Qualifications

• RN licensed with a BSN from an accredited program.
• Current RN license in applicable state; BLS for required states.
• One year RN or LPN experience preferred at Mayo Clinic.

Responsibilities

• Perform post-hospital follow-up calls for patients at risk of readmission.
• Conduct in-home visits for adult medical care coordination after discharge.
• Collaborate with primary care providers to develop and monitor care plans.
• Manage a defined panel of patients with ongoing follow-up and coordination.
• Educate patients about chronic conditions, treatments, and medications.
