Qualifications

• RN degree from a baccalaureate program or higher.

Responsibilities

• Conduct post-hospital follow-up calls for at-risk patients.
• Perform in-home visits for adult medical care coordination after discharge.
• Collaborate with primary care teams to develop and monitor care plans.
• Manage a defined panel with ongoing follow-up and coordination.
• Educate patients on chronic conditions and medications to promote engagement.
