Job summary

Mayo Clinic is seeking an experienced Registered Nurse (RN) to join a specialized Primary Care team in Minnesota. The role focuses on coordinating care for patients with chronic conditions transitioning from hospital to home, including post-hospital follow-up and in-home visits.

The RN will partner with primary care providers to develop individualized care plans, manage patient panels, and educate patients on their treatment plans and medications to improve outcomes and engagement.

Qualifications

• RN license in applicable state with BLS competency
• Experience in coordinating care for patients with chronic conditions preferred
• Strong communication and teamwork skills required

Responsibilities

• Post-hospital follow-up calls for at-risk patients.
• In-home visits for adult medical care coordination after discharge.
• Collaborate with primary care providers to implement care plans.
• Manage a defined panel of patients with ongoing follow-up and coordination.
• Educate patients on chronic conditions and medications to promote engagement.
