Job summary

ProMedica is seeking an RN Program Coordinator in the Pulmonary department to ensure collaborative care from hospital discharge through follow-up visits. You will coordinate the patient plan of care, provide education for procedures and testing, and review Health Risk Assessments with providers and patients.

You will monitor charts for test results, manage preoperative clearances with providers, and, when needed, triage calls with health advice and referrals.

Qualifications

• Current State license as a Registered Nurse.
• ACLS and BLS certification.
• 1 year of relatable or equivalent experience.

Responsibilities

• Coordinate patient care plan from hospital discharge through follow-ups.
• Educate patients and provide instructions for procedures and testing ordered by providers.
• Review patient Health Risk Assessment and monitor test results with provider and patient.
