Job summary

ProMedica's Ebeid Children's Hospital in Toledo is seeking an RN Program Coordinator to ensure seamless post-discharge care and coordinate the patient plan of care. You will educate families on procedures and review Health Risk Assessments with providers.

You will monitor charts for test results, review preoperative clearances, and may triage calls with health guidance and referrals as appropriate. A valid RN license, ACLS/BLS, BSN preferred, and EPIC experience are helpful.

Qualifications

• Current State license as a Registered Nurse (OH).
• One year of relatable clinical experience.
• ACLS and BLS certifications.
• BSN preferred; EPIC EMR experience.

Responsibilities

• Coordinate the patient plan of care from discharge through follow-ups.
• Provide education and instructions for procedures and testing.
• Review Health Risk Assessments with providers and patients.
• Monitor charts for test results and discuss with providers.
• Review preoperative surgical clearance requests; triage calls as needed.
