Baptist Health Princeton Hospital is seeking a Care Coordination Professional to collaborate with care teams, guide patients and families, and drive efficient discharge planning across inpatient, emergency and ambulatory settings.

The role focuses on identifying patients who benefit from care coordination, mentoring staff, and ensuring adherence to clinical standards while supporting patient-first goals.

Qualifications

• RN graduate from an approved nursing program; licensure required.
• Masters in Social Work (MSW), Mental Health (MHC) or Marriage and Family Therapy (MFT) from an accredited program; required.

Responsibilities

• Lead continuity of care across inpatient, emergency and ambulatory settings; coordinate discharge planning.
• Collaborate with mental health counselors, social workers and UR nurses to advocate and support patient needs.
• Mentor care coordination team through guidance, education and motivation.
• Monitor team performance toward discharge plans and adjust as needed for high-risk patients.
• Build relationships with hospital administration, medical staff and managed care partners.
• Ensure compliance with clinical standards and regulatory requirements.
