Job summary

Baptist Health seeks a Nurse Navigator to serve as the primary clinical contact for specialty care patients and families. You will coordinate care across the continuum, guiding patients from diagnosis through treatment, discharge, and follow-up, and act as educator, advocate, and care coordinator to support informed decisions and adherence.

Under the direction of the Practice Manager, you will work to remove barriers to timely care, improve system flow, and enhance patient and provider

Qualifications

• BSN required.
• Minimum of three years of job-related experience.
• Current Kentucky or Compact State RN license.
• BLS certification via the American Heart Association.

Responsibilities

• Serve as the primary clinical point of contact for specialty care patients and their families.
• Coordinate care across the continuum from diagnosis to follow-up.
• Educate, advocate, and coordinate with patients and providers to ensure timely, seamless transitions.
