Job summary

Meritus Health is seeking a Part Time Care Continuum Navigator (Registered Nurse) in Maryland. The role collaborates with ED, inpatient care management, physicians, and community partners to coordinate high-risk patient care across the continuum and improve outcomes.

Identify frequent ED users, assess drivers of utilization, and develop individualized care plans to promote health, independence, and appropriate resource use.

Qualifications

• ASN with a minimum of 3 years acute care experience and/or BSN with at least 2 years in acute care or utilization roles.
• Current Maryland RN license, or compact-state license with Maryland eligibility and required verification.
• Care/case management certification within 5 years of employment.

Responsibilities

• Coordinate transitions of care with ED, inpatient care management, physicians, and community partners.
• Identify high-risk patients and develop individualized care plans.
• Assess clinical, behavioral, social, and environmental factors affecting utilization.
• Create patient-centered care plans with families and care teams.
• Ensure continuity across inpatient, outpatient, and post-acute settings.
• Facilitate ED discharge planning with follow-up and community resources.
• Partner with care management teams to reduce avoidable utilization.
• Advocate for patients, promoting safe and appropriate care.
• Monitor progress and evaluate care coordination using quality metrics.
• Maintain accurate documentation and regulatory compliance.
