Job summary

Trinity Health Mid-Atlantic is seeking a Part-time Acute Care Manager to coordinate and manage care, ensure handover, and reduce fragmentation across the care continuum. This on-site per diem role requires an RN with clinical expertise and strong communication with interprofessional teams.

The role emphasizes documentation accuracy, optimizing level-of-care decisions, and collaboration with admission teams to ensure smooth patient transitions. BLS as a minimum licensure, ACLS preferred.

Qualifications

• Bachelor's degree in nursing preferred.
• 3–5 years of direct patient care or case management experience in a similar size hospital preferred.
• Acting independently with ability to analyze clinical, financial and social situations, and directing the healthcare team.

Responsibilities

• Participates in Performance Projects (A3) to be determined by Manager/Director.
• Ensures appropriate clinical documentation to support level of care and analyzes patient status/history for gaps.
• Serves as a consultant and educator with medical staff regarding status, payer requirements, and utilization of resources.
• Assures appropriate sequencing of care/interventions for optimal results and smooth transition along hospital care continuum.
• Coordinates level of care appropriateness with admission status determination.
• Maintains knowledge of healthcare economics and applies to daily practice.
