Job summary

Empath Health’s Tidewell Hospice seeks an RN for the Care Center on a PRN basis with day shifts in Sarasota/Bradenton/Venice area. The role requires strong clinical assessment, care planning, and patient education to support families and the interdisciplinary team.

You will provide age-specific nursing care in hospice/post-acute settings, administer medications, document care, and collaborate with the IDT while maintaining the organization’s Full Life Care philosophy. Florida RN license required.

Qualifications

• Associate’s degree (A.A.) or equivalent from two-year college or technical school.
• Minimum of 1 year of current active nursing experience; with terminally ill care preferred.
• An equivalent of education and experience may be considered.

Responsibilities

• Obtain patient history and perform age-specific assessments.
• Develop Plan of Care with patient, family, IDT, attending physician and partnering facility.
• Administer, monitor and document medications via all routes.

Schedule: 8am -8:30pm, Candidates will assist at Lakewood Ranch, Sarasota, Bradenton, and Venice Hospice houses.**What you'll Do*** Supports and carries out the organization’s mission, vision, and values on a consistent basis.* Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.* The RN will identify patient/family goals, develop a Plan of Care with patient, family, IDT, attending physician and/or partnering facility and continually evaluate progress towards goals.
