Job summary

Tidewell Hospice, part of Empath Health, is seeking an RN to join our Florida hospice team serving Lakewood Ranch, Sarasota, Bradenton and Venice. You will perform age-specific assessments, develop and monitor a Plan of Care with the IDT, and coordinate referrals and interventions to support patients and families.

You’ll administer medications, educate patients and families, and track progress according to policy.

Qualifications

• Minimum 1 year of active nursing experience, preferably in terminally ill care.
• Current Florida Registered Nurse license.
• Valid Florida driver’s license.
• Experience in acute or post-acute care setting preferred.

Responsibilities

• Perform age-specific history and physicals, psychosocial and spiritual assessments and initiate referral, intervention and collaboration with IDT.
• Develop and update the Plan of Care with patient, family, IDT and attending physicians.
• Administer, monitor and document medications using age-appropriate practices and provide patient/family teaching.
• Identify learning needs and tailor education to patient and caregiver capabilities.

Minimum of 1 year of current active nursing experience; with experience in caring for terminally ill patients or those with life threatening illnesses preferred; prefer one (1) year experience in an acute or post acute care setting.
