Job summary

Tidewell Hospice, part of Empath Health, is hiring an RN Case Manager in Port Charlotte, FL. You will begin at admission by creating and maintaining a plan of care, coordinating patient and family interactions throughout the hospice experience.

The role requires strong listening, observation, and compassionate nursing in emotionally intense settings. Applicants must have a Florida RN license and at least one year of acute-care hospital experience; hospice experience is preferred.

• Current Florida RN license
• Minimum one year experience in an acute-care hospital environment; hospice RN experience preferred

Responsibilities

• Create and maintain the plan of care starting at admission, coordinating patient and family interactions with Tidewell Hospice for the duration of the hospice experience
• Review and update the care plan, physician orders, and pharmacy profile; educate patients and families about disease process and palliative interventions and safety practices

When a patient and loved ones think of Empath Health Hospice, they will think first of the RN Case Manager. The case manager must be able to listen, observe, communicate and sensitively provide nursing services in emotionally intense situations.* The RN Case Manager reviews and updates the care plan, physician orders, pharmacy profile and other records.
