Elara Caring is hiring a Registered Nurse Behavioral Health for the Hartford area to deliver home-based care. This full-time daytime role offers strong benefits and flexibility, helping patients stay home where they prefer to be.

You will assess, coordinate, and update care plans, educate families, and support staff across settings while ensuring safety and high-quality outcomes. Prior experience in home health is valued.

Competenze

• Active RN license in the state.
• Graduate of an accredited Nursing program.

Mansioni

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
