Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health for a full-time day position serving the New Milford and Danbury area. You will assess patients, develop care plans, and educate families while supporting behavioral health staff.

The role offers flexible hours and community-based care to keep clients at home where they want to be. Requirements include an active RN license and 1+ year of clinical nursing experience; home health is preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associates or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.

Responsibilities

• Assess patients' physical, emotional, social, and environmental needs.
• Manage caseload and update individualized care plans.
• Complete initial assessments and ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
