Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health for full-time day shifts in the New Milford and Danbury, CT area. You’ll assess, coordinate, and manage patient care in homes, with on-call responsibilities and holidays as needed.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/ Diploma/ Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical setting.

Responsibilities

• Assess patients’ physical, emotional, social, and environmental needs.
• Manage a caseload and update individualized care plans.
• Complete initial assessments and ongoing evaluations during visits.
• Monitor patient progress and communicate changes to physicians and care team.
• Update care plans as needs evolve; educate patients and families about plan of care and behavioral health support.
• Teach behavioral health care to staff in partner care settings; supervise aides and LPNs.
• Provide end-of-life support and bereavement assessments.
