Job summary

Elan Skilled Nursing and Rehab is seeking an RN Assessment Coordinator (RNAC) in Scranton, PA. This hourly position starts at $42.00 per hour and includes benefits such as 403(b), medical/dental/vision, and paid time off.

The RNAC will oversee MDS assessments, manage medical records, and coordinate care plans with the interdisciplinary team to ensure regulatory compliance. The ideal candidate will have a PA nursing license, 3–5 years of RN experience (preferably in geriatrics), and knowledge of

Qualifications

• Registered Nurse with at least three to five years experience.
• Must maintain current Basic Cardiopulmonary Life Support (BCLS).
• Strong organizational and interpersonal skills.

Responsibilities

• Oversee and coordinate MDS assessments for Medicaid/Medicare PPS.
• Coordinate the interdisciplinary team to develop resident care plans.
• Maintain and audit medical records and census data for payer accuracy.
• Ensure compliance with regulatory requirements and complete required documentation.
