Job summary

Linwood Nursing and Rehabilitation Center in Scranton, PA seeks an experienced Registered Nurse Assessment Coordinator (RNAC) to support regulatory compliance, resident-centered care and survey readiness. The role emphasizes accurate resident assessment and care planning with knowledge of MDS, RAI, and CAAs.

The RNAC will coordinate OBRA and Medicare/managed care assessments, participate in committees, and ensure adherence to federal, state, and local guidelines for the facility.

Qualifications

• Associate or Bachelor Degree in Nursing preferred.
• Previous experience in completion of Resident Minimum Data Sets and Care planning preferred.
• Basic computer knowledge is essential.

Responsibilities

• Contributes to regulatory compliance and survey readiness through accurate resident assessment and care planning.
• Coordinates scheduling of OBRA and Medicare/managed care assessments and care plan reviews.
• Maintains knowledge of MDS/RAI and CAAs for care coordination within Pennsylvania guidelines.
• Communicates Medicare/managed care coverage status to residents and responsible parties as needed.
• Participates in committees as directed.
• Demonstrates strong knowledge of Pennsylvania case mix and PDPM requirements for managed care coordination.
