Job summary

Twin Lakes Rehabilitation and Healthcare Center in Greensburg, PA is seeking a Full Time Registered Nurse Assessment Coordinator (RNAC) to lead our nursing team's RAI process and resident assessments. You will ensure compliance with state and federal regulations and coordinate MDS activities under Medicare/Medicaid guidelines.

You will provide education to the interdisciplinary team, participate in surveys, and help maintain a robust quality assurance program while advancing professional

Qualifications

• RN with long-term care assessment experience.
• Knowledge of state and federal regulations.
• At least 1 year RNAC experience in a skilled nursing facility (MDS).
• Proficient in PA CMI.
• Medicare billing knowledge and experience.
• Knowledge of PPS (PDPM).
• Must complete post-offer background check and drug screen.

Responsibilities

• Conduct and coordinate resident assessments per state requirements and facility goals.
• Coordinate RAI processes under Medicare/Medicaid and other payer program rules.
• Develop, implement, and maintain an ongoing quality assurance program for resident assessment.
• Participate in facility surveys conducted by government agencies.
• Monitor QI/QM reports to trigger corrective actions as needed.
• Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.
