Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to provide in-home patient assessments, treatment, and support. The role includes crisis management, pronouncement when needed, and collaboration with the patient’s physician to plan care.

You will document progress notes, communicate changes in patient needs, and ensure timely, compliant care while delivering compassionate service to patients and families in their homes.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License with reliable transportation and auto liability insurance
• Two years of RN experience preferred in healthcare/clinical setting

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including pronouncement of patient death when needed
• Develop and revise nursing care plans with patient physician and IDG
• Utilize excellent customer care skills with patients, families and internal/external staff
• Prepare clinical and progress notes showing progress toward established goals
• Inform physician and other personnel of changes in patient needs and outcomes of intervention
• Timely submission of all documentation
• Perform other duties as required
• Ensure compliance with local, state and federal laws and Medicare regulations

What You Must Have:
