Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to deliver compassionate home-based care, perform patient assessments, treatment, and support, including pronouncement of death when needed.

You will develop and revise nursing care plans with physicians, document progress, communicate changes to the IDG, and ensure timely, compliant clinical notes in a respectful, patient-centered environment.

Qualifications

• Must be a graduate of an accredited nursing program (Diploma/Associate/Baccalaureate).
• Current State Registered Nurse License.
• Valid drivers license and reliable transportation.
• Two years of nursing experience preferred in healthcare/clinical settings.

Responsibilities

• Provide home visits for patient assessment, treatment, and support.
• Develop and revise nursing care plans with the patients physician and IDG.
• Use excellent customer care and proper phone etiquette.
• Prepare progress notes showing goals progress.
• Inform physician of changes in patient needs and outcomes.
• Submit documentation in a timely manner.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.

What You Must Have:

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
