Job summary

Heart to Heart Hospice in Austin, TX seeks an on-call Registered Nurse to provide in-home patient assessment, treatment and support. This role includes pronouncement when needed and crisis management, with collaboration on care plans alongside physicians and the IDG.

The ideal candidate has an RN license, reliable transportation, and at least 2 years of RN experience in healthcare settings. The position offers full-time hours and competitive benefits.

Qualifications

• Graduate of accredited Diploma, ADN or BSN program.
• Current State RN license.
• Driver's license with reliable transportation and active auto liability insurance.
• Two years RN experience preferred.

Responsibilities

• Provide home visits for patient assessment, treatment and support, including end-of-life pronouncement when needed.
• Develop and revise nursing care plans with physician and IDG for comprehensive patient and family care.
• Maintain excellent customer care with internal and external contacts, including proper phone etiquette.
• Prepare clinical and progress notes showing progress toward goals.
• Inform physicians of changes in patient needs and outcomes and assess family response to care.
• Submit documentation in a timely manner.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and personnel policies.

What You Must Have:

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
