Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to provide compassionate home visits in the League City area. You will assess and treat patients, support families, and respond to urgent clinical needs as part of a coordinated care team.

The role requires a valid RN license, a current driver's license and reliable transportation. Two years of RN experience in healthcare or clinical settings is preferred, with a focus on high-quality, compliant care.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of RN experience preferred in healthcare/clinical setting.
• Must have a valid driver's license with reliable transportation and auto liability insurance.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including crisis management and potential pronouncement of death.
• Develop and revise nursing care plans with patient, family, and physician as needed.
• Maintain excellent customer care with internal and external stakeholders and use proper phone etiquette.

What You Must Have:

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
