Job summary

Amedisys Admissions Registered Nurse for the Greater Worcester County area performs start of care assessments, develops and implements the plan of care, and coordinates care with staff, physicians, and families to promote safe, effective home health.

This full-time day role offers a $15,000 sign-on bonus and compensation between $91,700 and $114,500 annually, with comprehensive benefits, paid time off, and growth opportunities with a trusted national home health provider.

Qualifications

• Current RN license valid in Massachusetts.
• One year of clinical RN experience; VP approval if less.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs start of care comprehensive assessment and develops the plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care according to the plan of care and agency policies.
• Helps achieve continuity of patient care by communicating with staff, physicians, families and resources.
• Initiates referrals when indicated.
• Promotes patient health through teaching and rehabilitative measures.

Responsibilities

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.

• One year of clinical experience as a n RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
