Job summary

Empath Health's Suncoast Hospice in Clearwater, FL is seeking an Admissions RN to work Sunday through Wednesday, 10am–8pm, coordinating patient admissions across long-term care, home, hospital, and hospice settings.

You will educate families, assess needs after terminal diagnoses, and contribute to a compassionate team delivering Full Life Care. Florida RN licensure is required; BSN/MSN preferred; 1 year acute-care experience; hospice experience preferred.

Qualifications

• Must possess a clear and current Florida Licensure as RN.
• Minimum one year experience in an acute-care Hospital / In-patient environment providing direct patient care.
• Previous experience as a Hospice RN preferred.

Responsibilities

• Coordinates patient admissions activities in the long term care facility, patient home, hospital, assisted living facility and hospice house settings.
• Educates patient, family and caregivers regarding disease process and decline, palliative interventions, care giving, dying process, ethical concerns and safety practices.
• Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated Admissions RN to work with us in Pinellas county providing exceptional care on the following schedule: Sunday - Wednesday 10am - 8pm
