Job summary

Suncoast Hospice, a part of Empath Health, is seeking an Admissions RN to serve in Pinellas County on a Tue-Fri, 9:00 a.m. to 7:00 p.m. schedule, delivering compassionate end-of-life care.

The role emphasizes coordination of admissions across settings and education for patients and families. Requirements include a clear Florida RN license; BSN or MSN is preferred. Prior hospice or acute-care inpatient experience is highly valued as you join a mission-driven team dedicated to Full Life Care for

Qualifications

• Must possess a clear and current Florida RN licensure.
• Minimum one year experience in an acute-care hospital/in-patient environment providing direct patient care; hospice RN experience preferred.

Responsibilities

• Coordinates patient admissions activities across long term care facilities, patient home, hospital, assisted living facilities, and hospice house settings.
• Educates patient, family and caregivers regarding disease processes, palliative interventions, caregiving, end-of-life considerations, ethics and safety practices.
• Provides initial assessment of the impact of terminal diagnosis on physical, functional, psychosocial and environmental needs and activities of daily living.

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated Admissions RN to work with us in Pinellas county providing exceptional care on the following schedule: Tuesday-Friday 9a-7p.

What you'll Do:

• Coordinates patient admissions activities in the long term care facility, patient home, hospital, assisted living facility and hospice house settings.

• Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.

What You’ll Need:
