Job summary

Suncoast Hospice, part of Empath Health, seeks a caring Admissions RN to cover Pinellas County on Tue-Fri 9am-7pm, delivering exceptional end-of-life care and coordinating patient admissions across facilities.

You will assess needs, educate patients and families, and work with a multidisciplinary team to ensure seamless transitions and compassionate, Full Life Care.

Qualifications

• Must possess a clear and current Florida Licensure as RN.
• BSN or MSN preferred.

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated Admissions RN to work with us in Pinellas county providing exceptional care on the following schedule: Tuesday-Friday 9a-7p.

What you'll Do:

• Coordinates patient admissions activities in the long term care facility, patient home, hospital, assisted living facility and hospice house settings.
• Educates patient, family and caregivers regarding disease process and decline, palliative interventions, care giving, dying process, ethical concerns and safety practices.
• Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.

What You’ll Need:

• Must possess a clear and current Florida Licensure as RN. BSN or MSN preferred.
• Minimum one year experience in an acute-care Hospital / In-patient environment providing direct patient care; previous experience as a Hospice RN preferred.
