Job summary

Advanced Health Care is seeking an experienced RN Admissions Nurse to streamline the admission process for new residents and ensure a smooth transition into our facility with high-quality nursing care.

You will assess medical histories, coordinate with physicians and families, complete required paperwork, administer medications as prescribed, and support discharge planning under the Director of Nursing in Meridian, Idaho. Strong clinical skills and clear communication are essential.

Qualifications

• Active RN license in Idaho.
• 2+ years of admissions/intake experience in healthcare.
• Strong assessment, critical thinking and clinical judgment.
• Knowledge of regulatory requirements for admissions/documentation.
• Ability to build rapport with residents and families.
• Proficient in electronic medical record systems.

Responsibilities

• Conduct thorough assessments of incoming residents to gather medical history and identify care needs.
• Collaborate with physicians and nursing staff to develop individualized admission plans.
• Communicate effectively with residents, families, and caregivers about admissions and services.
• Complete admission paperwork accurately and obtain required documentation.
• Coordinate with dietary and therapy to arrange assessments for new admissions.
• Administer medications and treatments as prescribed by the physician.
• Monitor vital signs and report changes to the care team.
• Provide emotional support to residents and families during admissions.
