Job summary
Hebrew Center for Health & Rehabilitation in West Hartford, CT is seeking an Admissions Nurse to lead the admission process, assess and coordinate transitions for new residents, and ensure a smooth entry with compassionate care. Qualifications
- Valid state RN nursing license.
- Experience in Long-Term Care setting preferred.
- Compassionate and empathetic approach to patient care.
- Ability to work with aging and chronically ill residents in a multidisciplinary team.
Responsibilities
- Conduct comprehensive nursing assessments for potential residents.
- Collaborate with healthcare teams to review medical histories and care needs.
- Assist in developing individualized care plans for new admissions.
- Coordinate and communicate care plans with residents, families, and healthcare professionals.
- Educate residents and families about facility services, procedures, and care expectations.
- Ensure compliance with regulatory requirements during the admission process.
