Responsibilities

As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, while continuously evaluating your performance and making necessary improvements.

• Perform the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identify and reassess the patient's physical, psychosocial, and environmental needs at least every 15 days.
• Initiate and coordinate the plan of care, maintaining accurate clinical notes.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with the patient/family, physician, and other team members daily.

Education and Experience

Experience Requirements:

• Minimum of one year of clinical experience.

License Requirements:

• Current RN licensure in the state of practice.
• Current CPR certification.
• Valid driver's license, insurance, access to a dependable vehicle or public transportation.
