We are hiring for an RN Case Manager in Hospice. The position offers a $2,500 Sign-on Bonus and is located in the Lafayette, TN area.

Responsibilities:
  • Make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify and reassess the physical, psychosocial, and environmental needs of patients and families, at least every 15 days.
  • Initiate and coordinate the plan of care, maintaining accurate clinical notes.
  • Document problems, goals, interventions, and patient/family responses.
  • Collaborate with the patient/family, attending physician, and other team members daily.
Experience Requirements:
  • At least one year of clinical experience.
License Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Valid driver’s license, insurance, access to a dependable vehicle or public transportation.
Additional State Specific Requirements:
  • No additional requirements.
