Job Title: RN Case Manager - Hospice

We are hiring for an RN Case Manager in Hospice with a $2,500 Sign-on Bonus. This is a full-time position at HighPoint Hospice in Gallatin, TN, part of LHC Group. As an RN Case Manager, you will:

  • Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
  • Evaluate personal and professional performance regularly and implement improvements.
  • Perform initial nursing evaluations within 48 hours of referral to determine eligibility for hospice services.
  • Identify and reassess the patient's physical, psychosocial, and environmental needs at least every 15 days.
  • Initiate and coordinate the plan of care, maintaining accurate clinical notes.
  • Document problems, goals, interventions, and patient/family responses.
  • Collaborate daily with the patient/family, attending physician, and IDG team.
Experience & License Requirements
  • Minimum of one year of clinical experience.
  • Current RN licensure in the state of practice.
  • Current CPR Certification.
  • Valid driver’s license, vehicle insurance, and access to transportation or public transit.
