We are hiring for an RN Case Manager in Hospice. Position is Full Time. $2,500 Sign-on Bonus!

Responsibilities

As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, continuously evaluating your performance and making necessary adjustments to improve care quality and productivity.

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient's and family's physical, psychosocial, and environmental needs, and re-assess as needed, at least every 15 days.
• Initiate and coordinate the plan of care, maintaining accurate clinical notes regarding patient condition.
• Document problems, goals, interventions, and patient/family responses to hospice care.
• Collaborate with the patient/family, attending physician, and IDG team to provide comprehensive care daily.

Education and Experience

Experience Requirements:

• At least one year of clinical experience.

License Requirements:

• Current RN licensure in the state of practice.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
