Millennium Post Acute Rehabilitation in West Columbia, SC, is seeking a full-time Registered Nurse (RN) for 12-hour day shifts with a sign-on bonus. You will collaborate closely with the DON, NP, and physicians to ensure high-quality resident care and accurate clinical documentation.

• RN license required.
• Experience in skilled nursing or long-term care preferred.
• Ability to monitor patient conditions and collaborate with physicians and NP.

Responsibilities

• Manage nursing units with DON; supervise staff and ensure clinical outcomes.
• Monitor patient conditions with NP to provide high quality care.
• Observe symptoms and communicate findings to physicians for timely decisions.
• Collaborate with physicians and nurses to create individualized care plans.
• Perform routine procedures, record charts, and administer medications per orders.
• Provide treatments and adjust medications as per physician guidance.

Job Responsibilities:

• Work very closely with DON to manage units, own outcomes for clinical systems, supervise nursing staff, etc.

• Work closely with NP to monitor patient’s condition and assess their needs to provide the best possible care and advice

• Observe and interpret patient’s symptoms and communicate them to physicians

• Collaborate with physicians and other nurses to devise individualized care plans for patients

• Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients’ charts

• Adjust and administer patient’s medication and provide treatments according to physician’s orders
