Qualifications

• Current license to practice as an Advanced Practitioner in the state performing duties.
• 2 years clinical work experience.
• Nephrology experience is a strong plus.
• Excellent written and verbal communication skills.

Responsibilities

• Complete CDMVs and ESKD rounding, standalone or integrated in pathways.
• ESKD care management including activation, depression, transitions of care.
• Educate patients and families; coordinate with nephrologists and care teams.
• Order tests, diagnose, treat, and document in EMR; promote preventative care.
