Job summary

The University of Miami Health System seeks a Full-Time Rheumatology Nurse Navigator 1 to provide proactive clinical navigation for new and complex rheumatology patients throughout the care continuum.

You will perform pre-visit chart preparation, identify barriers to care, educate patients, and coordinate cross-disciplinary care with APRN, physicians, nurses, CBOR staff, pharmacies, infusion services, and external partners. Florida RN license required.

Qualifications

• Bachelor’s degree in relevant field.
• Valid State of Florida RN license.
• Minimum 2 years of relevant work experience.
• Ability to communicate effectively; strong interpersonal skills.
• Proficiency in Microsoft Office.

Responsibilities

• Prepare clinical charts for new rheumatology patients before the initial visit.
• Identify missing records and coordinate retrieval of information.
• Coordinate resources to support timely access and continuity of care.
• Provide high-level care coordination for complex rheumatology patients.
• Lead workflow of Patient Navigators and coordinate cross-functional teams.
• Educate patients and caregivers on testing, referrals and follow-up steps.
• Monitor navigation worklists and ensure closed-loop resolution.
