Job summary

University of Miami is seeking a Nurse Navigator 1 (H) to be the initial contact for patients entering the health system, ensuring timely scheduling of the first appointment and coordinating care after the first visit. The Nurse Navigator 1 (H) serves as a liaison between patients, families, caregivers, and the multidisciplinary care team.

The incumbent demonstrates a commitment to quality patient care, educates patients about their treatment plan, and supports them throughout the care

Qualifications

• Education: Bachelor’s degree in a relevant field.
• Certification: Valid State of Florida RN license.
• Experience: Minimum 2 years of relevant work experience.
• Skills: Interpersonal, communication, data analysis, MS Office.

Responsibilities

• Triages new patient appointments and ensures timely scheduling.
• Reviews outside medical records for scheduling accuracy.
• Assesses barriers to care and refers to support services.
• Educates on treatment plans and coordinates care across the continuum.
• Collaborates with multidisciplinary team to outline best treatment.
• Mentors other professionals as a preceptor when needed.
