Qualifications

• Minimum education: high school diploma or GED.

Responsibilities

• Under close supervision, assists Case Managers with scheduling, reception, and records maintenance.
• Coordinate patient referrals and follow-up after provider visits.
• Assist with referrals, scheduling diagnostic studies, clinic appointments, and travel logistics.
• Support admissions, orders, and electronic forms with nursing staff.
• Maintain patient charts and ensure accuracy before charting in the Medical Records department.
• Use scheduling and electronic health record systems to track patient follow-ups.
