Health Risk Assessment

Health Risk Assessment Step by Step Instructions:

1. Helping Hand Donation Form: This is to be completed and given to your manager for payroll deductions and participation in the Helping Hand Fund.
2. 403b information guide: Information about the 403b, including enrollment paperwork. Complete enrollment paperwork online (directions are within the guide).
3. After 1 year of employment, Goodwill will match your contribution up to 4%. You may complete your enrollment online at principal.com or by calling 1-800-547-7754.
4. Enrollment Documents are also available for the AbilityOne contracts for the 401a. Please complete if you are at either of the Ability One locations.
