Job summary

Qualifications

• BA/BS degree in social work, criminal justice, psychology, counseling or related field required.
• Must have a valid driver's license and access to reliable transportation.

Responsibilities

• Provides coordination for referrals and maintains referral agreements with community sources.
• Develops relationships with Police, Public Schools, Juvenile Probation, DCF and area service providers.
• Provides direct case management and develops plans to improve treatment completion and life functioning.
• Maintains case management records and documents in accordance with Medicaid/licensing.
• Determines eligibility for the Youth Diversion Team and ensures parental/youth agreement.
• Performs screenings and prepares summaries for the YDT.

Education
