Job summary

Elevate Care in Des Plaines, IL is seeking a compassionate Restorative Nurse (RN or LPN) to design and implement personalized restorative care plans that promote residents' independence and quality of life.

You will monitor progress, provide therapeutic activities, educate residents and families, and collaborate with nursing and therapy teams to ensure coordinated, person-centered care in a supportive environment.

Qualifications

• Active RN or LPN license.
• Experience in Restorative Nursing, Rehab, or Long-Term Care preferred.
• Strong assessment and critical-thinking skills.

Responsibilities

• Develop and implement Restorative Care Plans to promote independence.
• Monitor and evaluate resident progress and adjust plans as needed.
• Provide therapeutic activities to maintain or improve mobility and function.
• Educate residents and families on exercises, safety and recovery strategies.
• Collaborate with Nursing and Therapy teams to coordinate care.
• Document and record resident progress accurately.

Your Responsibilities:

• Develop & Implement Restorative Care Plans - Work with residents and the healthcare team to promote functional independence.
• Monitor & Evaluate Resident Progress - Track the effectiveness of restorative interventions and adjust care plans as needed.
• Provide Therapeutic Activities - Assist residents with exercises, mobility, and functional tasks to maintain or improve their abilities.
• Educate Residents & Families - Provide guidance on exercises, safety, and strategies to enhance recovery.
• Collaborate with Nursing & Therapy Teams - Ensure coordinated care across all disciplines.
• Document & Record Resident Progress - Maintain accurate and up-to-date records of care and progress.

Qualifications:

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
• Experience in Restorative Nursing, Rehab, or Long-Term Care (Preferred).
• Strong Assessment & Critical Thinking Skills.
• Excellent Communication & Teamwork Abilities.
