Elevate Care is hiring a Restorative Nurse (RN or LPN) to develop and implement restorative care plans, promoting residents' independence in daily activities.

Responsibilities include monitoring progress, providing therapeutic exercises, educating families, and coordinating with nursing and therapy teams to ensure comprehensive care.

Qualifications

Your Responsibilities:

Develop & Implement Restorative Care Plans – Work with residents and the healthcare team to promote functional independence.

Monitor & Evaluate Resident Progress – Track the effectiveness of restorative interventions and adjust care plans as needed.

Provide Therapeutic Activities – Assist residents with exercises, mobility, and functional tasks to maintain or improve their abilities.

Educate Residents & Families – Provide guidance on exercises, safety, and strategies to enhance recovery.

Collaborate with Nursing & Therapy Teams – Ensure coordinated care across all disciplines.

Document & Record Resident Progress – Maintain accurate and up-to-date records of care and progress.

Qualifications:

Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) License

Experience in Restorative Nursing, Rehab, or Long-Term Care (Preferred).

Strong Assessment & Critical Thinking Skills.

Excellent Communication & Teamwork Abilities.
