Qualifications

• Active Pennsylvania nursing license.

Responsibilities

• Develop individualized care plans for restorative care, focusing on mobility, strength and functional recovery.
• Assess physical, emotional, and social needs of patients and communicate with patients, families and healthcare teams.
• Collaborate with occupational, physical and speech therapists to implement treatment plans that promote recovery and independence.
• Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
• Maintain accurate records of patient care, treatment plans and progress notes.
• Participate in quality improvement initiatives and contribute to evidence-based practices.
