Qualifications

• Provide skilled nursing care within the restorative program
• Conduct resident assessments and develop personalized care plans
• Implement rehabilitation programs to promote independence and mobility
• Identify and address health issues, and educate residents and families
• Maintain accurate records of care and services provided
• Collaborate with the interdisciplinary care team to implement care plans
• Provide education and support to residents, families, and caregivers
