Premier Washington Health Center in Washington, PA is seeking a dedicated Restorative Nurse to join our team of healthcare professionals. This full-time role focuses on restorative care to improve mobility, strength, and independence for residents.

You will collaborate with therapists, monitor progress, document treatment plans, and participate in quality improvement initiatives to deliver exceptional patient care in a supportive environment.

Competenze

• Current PA nursing licensure required.
• Experience in restorative nursing preferred.

Mansioni

• Develop and implement individualized restorative care plans to improve mobility and functional abilities.
• Assess physical, emotional, and social needs and coordinate with families and care teams.
• Collaborate with OT, PT, and speech therapy to promote recovery and independence.

Restorative Nurse Responsibilities:

• Develop and implement individualized care plans for patients in a restorative care setting, focusing on improving mobility, strength, and functional abilities
• Assess and address the physical, emotional, and social needs of patients, communicating with patients, families, and healthcare teams to ensure comprehensive care
• Collaborate with occupational therapists, physical therapists, and speech therapists to develop and implement treatment plans that promote recovery and independence
• Monitor and report patient progress to ensure optimal outcomes and make adjustments to care plans as needed
• Maintain accurate and detailed records of patient care, treatment plans, and progress notes
• Participate in quality improvement initiatives and contribute to the development of evidence-based practices

Restorative Nurse Requirements:

• Current nursing licensure in the state of Pennsylvania
• Prior experience as a Nurse, preferably in restorative nursing
