Job summary

RYZE West is seeking a Restorative Nurse in Chicago, IL. This full-time role supports residents by maximizing independence and managing restorative care plans.

You will work with administration, nursing, therapy, and other departments to implement programs and ensure high-quality care while maintaining accurate documentation. Qualifications include an IL RN or LPN license and at least 1 year in restorative care, with training in rehabilitative practices.

Qualifications

• Must hold RN or LPN license issued by Illinois Board of Nursing.
• Minimum of 1 year of experience in restorative care.
• Training in rehabilitative and restorative therapy practices.
• Experience in long-term care preferred.

Responsibilities

• Plans, develops, organizes, implements, evaluates, and directs restorative care services and programs in accordance with regulations governing nursing facilities.
• Collaborates with administration, medical/nursing staff, therapy, and related departments for planning.
• Maintains restorative care standards.
• Assists QA committee in action plans for deficiencies.
• Directs restorative care services and performs necessary administrative duties.
• Interprets policies to staff, residents, families as needed.
• Assists HR in recruitment of restorative care personnel.
• Provides direct nursing care as needed.
• Ensures restorative care notes are informative and reflect care and resident response.
• Develops and participates in in-service education and orientation for staff.
