RYZE West is seeking a Restorative Nurse in Chicago, IL. This full-time role supports residents by maximizing independence and managing restorative care plans.
You will work with administration, nursing, therapy, and other departments to implement programs and ensure high-quality care while maintaining accurate documentation. Qualifications include an IL RN or LPN license and at least 1 year in restorative care, with training in rehabilitative practices.

As a Restorative Nurse, you will play a vital role in maximizing the resident's existing abilities, emphasizing independence, and minimizing the negative effects of disability. This full-time opportunity in Chicago, Illinois.
Plans, develops, organizes, implements, evaluates, and directs restorative care services and programs in accordance with current rules, regulations, and guidelines that govern nursing care facilities.
Meets with administration, medical and nursing staff, therapy department, and other related departments in planning restorative care.
Develops and maintains restorative care standards.
Assists the Quality Assessment and Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
Assumes the authority, responsibility, and accountability of directing restorative care services.
Performs administrative duties such as completing medical forms, reports, evaluations, studies, charting, and other tasks as necessary.
Interprets restorative care policies and procedures to personnel, residents, family members, and others as necessary.
Assists the Administrator and/or HR Director in the recruitment and selection of competent restorative care personnel.
Provides direct nursing care as necessary and as department needs may dictate.
Ensures that all restorative care notes are informative and descriptive of the treatment provided and of the resident's response to the care.
Develops and participates in programs designed for in-service education, on-the-job training, and orientation classes for newly assigned personnel.
Other duties as assigned.
Registered Nurse or Licensed Practical Nurse credentialed from the Illinois Board of Nursing
Must have, as a minimum, one (1) year of experience in restorative care
Must have training in rehabilitative and restorative therapy practices
Previous experience in long-term care is preferred
